You did not mention the age of your G.maa .
You need to be more precise in order to get more precise answer .

As Drenthe suggests, age, motivation, and mental awareness, among other things will affect your grandmother's treatment options. With her current readings, more frequent testing will be needed to evaluate treatment. If her readings are routinely this high, and she is on a reasonable diet and not drinking regular sodas, insulin is usually the best first choice. It brings the glucose down reliably, rests the beta cells that are now very stressed, and allows other medications like metformin, Actos, or Byetta to be added with more success to aid her control.

Is she resisting treatment or not being seen by a physician? Her current medical care appears inadequate at best. Immediate treatment is recommended, preferably by an endocrinologist or diabetes clinic – she will feel much better when her glucose is closer to normal.

Keep in mind, that it is NORMAL to see the glucose rise over time as the beta cells that make insulin gradually fail. A couple of the medication classes mentioned above have been show to keep beta cells from failing, although they work much better before the glucose gets to be this high.

All the best,
John
