Diabetes Rising by Dan Hurley is now available at the Diabetes Mall.

Nearly 90 years after the discovery of insulin, with an estimated $116 billion spent annually on the medical treatment of diabetes in the United States, why is diabetes the one major cause of death that’s been relentlessly rising for a century? Diabetes Rising investigates why the nearly two dozen medications approved for type 2 (adult-onset) diabetes, and all the high-tech treatments for type 1 (juvenile-onset) diabetes, are failing to slow this modern pandemic of Western civilization. The book also profiles promising new approaches that are making significant strides toward preventing, curing, or dramatically improving treatment of the disease. Written by Dan Hurley, a regular contributor to the science section of the New York Times (and himself a type 1 diabetic for over 30 years).

Diabetes: Sugar Coated Crisis by David Spero, RN is now available at the Diabetes Mall.

We hear almost daily of the alarming increase in obesity and diabetes. But contrary to popular opinion, Type 2 Diabetes is not a medical problem so much as it is a social pandemic caused by toxic environments – high in stress and sugar, low in opportunities to exercise or feel good about yourself – and a lack of power.

Diabetes: Sugar Coated Criss describes the social sources of the toxic environment, including the stress and inequality built into our modern culture and the traumas and loss of community that make people vulnerable to illness.

The latest issue of the Diabetes Cyber Kitchen is up! Here's a snippet:

As you know Cyber Kitchen is a food column. Sometimes we expand our topic to talk about living a healthy lifestyle with diabetes as our partner, or mention various tips on exercise, but since summer is the season for a myriad of conferences and meetings in the diabetes community I thought it would be a nice change to sit out in the backyard or on the front porch, enjoy some of nature's summer bounty, and catch up on a little news from the sugar-free grapevine, sometimes known as the insulin injected engine. But, let's start with some food.
The latest issue of the Diabetes Cyber Kitchen is up! Here's a snippet:

I placed my groceries on the conveyor belt in the supermarket, the usual milk, eggs, pasta, a few plum tomatoes, lemons and limes and a variety of fresh greens. The kid working the register pushed through the packaged goods, tomatoes and fruit just fine. Then a perplexed grin came over his face as he held up a bunch of flat leaf parsley and asked “broccoli?” Thinking that he was joking I said of course it was broccoli, what else could it possibly be? Next came a nice big bunch of turnip greens. He looked at it and me and smiled “So you must really like broccoli?” Sure enough when I checked the bill, he had charged me for 2 bunches of … yup … broccoli! CAN SOMEONE PLEASE TELL ME WHAT’S GOING ON?

New Ask John Q&A

I have read your book, Pumping Insulin, and in it you recommend making basal rate changes 4-8 hours prior to any low or high blood sugars. I am currently on Humalog Lispro and many of the websites I have seen recommend 1, 2, and 3 hours prior to any lows or highs. Why is there such a big disparity in recommendations with Humalog insulin? I am just trying to understand the effects of my insulin better, your help is greatly appreciated.

J

———————–

Hi Jim,

Why we recommend that small basal changes be started 4 to 8 hours before the problem situation arises is easy to understand. Although Humalog, Novalog, and Apidra claim to be fast acting, they are really not. About half of their glucose lowering activity is seen in the first 2 to 2.25 hours, with the other half trailing out over the next 3 to 3.5 hours. Plus, when a basal rate is increased or decreased, the change in basal dose is not completed until the end of the first hour, with the glucose-lowering impact of that 1st hour’s basal change not ending until about 6 hours after it was started. So, if the basal rate is increased by 0.1 u/hr, that tenth of a unit is not fully delivered until an hour has passed, and, though small, its impact continues for another 5 hrs. So start early and end early to get the effect you want at the time you want.

The Juvenile Diabetes Research Foundation (JDRF) and BD (Becton, Dickinson and Company) (NYSE: BDX) announced today an innovative program aimed at improving the treatment of type 1 diabetes by developing novel insulin delivery products to enhance the use of insulin pumps.

Through the program, JDRF will support BD's research and development of new products that deliver insulin from a pump to a patient in either an infusion set or patch-pump configuration. Research indicates that there are significant opportunities to enhance pump therapy by improving convenience as well as minimizing pain, kinking, occlusions and site infections. An additional goal of the program is improving the speed at which insulin works. These enhancements are intended to improve how people with diabetes control their insulin therapy and have a positive impact on their overall level of glycemic control.

Meter/Software Combo Overcomes A Big Problem

One big problem with blood glucose meters has long been that you can’t convert the individual readings into patterns and trends so you can figure out what to do about them. Oh, sure, many meters have software that does this in one fashion or another but downloading the data into software is often difficult because the cable you need is unavailable or lost or left at home or the office. Even a bigger problem occurs when you take the meter to the doctor’s office and they don’t have a cable to use to download data to software for the doctor to analyze and make recommendations.

The answer to this problem is the newly released Bayer Contour USB. It plugs right in to a PC or Mac without the need for any cables and contains—built right in—software called Glucofacts Deluxe that collects your blood sugar test results.

New Cyber Kitchen Up

It was quite overwhelming to be 1 of the 12,000 people in attendance at the International Diabetes Federation’s (IDF) World Diabetes Congress in Montreal, Canada in October. For 5 days the Palais de Congress bustled like some colorful global bazaar with streams of people from over 150 nations, speaking different languages, some dressed in their native garb and all communicating in the common bond of diabetes.

IDF chose Canada for a few reasons, one of them being that Canada is the home of major breakthroughs in the field of diabetes. We all know about Banting and Best. I might not be here to write this article and some of us might not be here to read it if it weren’t for Sir Frederick Banting and Charles Best and their group of scientists discovering the use of insulin as treatment for diabetes mellitus. They gave us the gift of life in 1922 in Toronto. The world has changed quite a bit since then and although their discovery still remains a lifeline, the diabetes population has exploded beyond belief. Most recent numbers published in the 4th edition IDF Atlas in Montreal reveal that 285 million people worldwide live with diabetes and indications show that the numbers of people in low and middle income countries are increasing rapidly and that far more people of working age are being diagnosed with Type 2 diabetes than had been estimated.

Climbing Stairs and Wild Salmon: A Common Bond?

When you live on the 7th floor of a 12 story building, riding the elevator up is an automatic response. About a month ago we had a power outage that affected the building for 4 days. It’s an older building and someone on the 12th floor decided to have radiant flooring installed. It knocked out the building’s juice. So, instead of the usual thoughtless hopping on the elevator, the tenants of the house were struck with the fact that we actually had to rely on our bodies to carry us to our apartments. Being the type of building we are, most of us made the best of a potentially chaotic situation. We formed a group of floor captains (I was one) to see if any older or sickly people needed help or groceries. We lit battery operated candles in the stairwells, and on some floors people played music on portable radios, to make the hike more pleasant and give it some rhythm. At the end of the 4 days when things returned to normal and the elevators were up and running, many of us realized the benefits earned and learned from stair climbing.
