            <description><![CDATA[<p>I have read your book, Pumping Insulin, and in it you recommend making basal rate changes 4-8 hours prior to any low or high blood sugars. I am currently on Humalog Lispro and many of the websites I have seen recommend 1, 2, and 3 hours prior to any lows or highs. Why is there such a big disparity in recommendations with Humalog insulin? I am just trying to understand the effects of my insulin better, your help is greatly appreciated.</p>

<p>Hi Jim,</p>

<p>Why we recommend that small basal changes be started 4 to 8 hours before the problem situation arises is easy to understand. Although Humalog, Novalog, and Apidra claim to be fast acting, they are really not. About half of their glucose lowering activity is seen in the first 2 to 2.25 hours, with the other half trailing out over the next 3 to 3.5 hours. Plus, when a basal rate is increased or decreased, the change in basal dose is not completed until the end of the first hour, with the glucose-lowering impact of that 1st hour’s basal change not ending until about 6 hours after it was started. So, if the basal rate is increased by 0.1 u/hr, that tenth of a unit is not fully delivered until an hour has passed, and, though small, its impact continues for another 5 hrs. So start early and end early to get the effect you want at the time you want.</p>

Oh, sure, many meters have software that does this in one fashion or another but downloading the data into software is often difficult because the cable you need is unavailable or lost or left at home or the office. Even a bigger problem occurs when you take the meter to the doctor’s office and they don’t have a cable to use to download data to software for the doctor to analyze and make recommendations.</p>

            <description><![CDATA[<p>It was quite overwhelming to be 1 of the 12,000 people in attendance at the International Diabetes Federation’s (IDF) World Diabetes Congress in Montreal, Canada in October. For 5 days the Palais de Congress bustled like some colorful global bazaar with streams of people from over 150 nations, speaking different languages, some dressed in their native garb and all communicating in the common bond of diabetes.</p>

<p>IDF chose Canada for a few reasons, one of them being that Canada is the home of major breakthroughs in the field of diabetes. We all know about Banting and Best. I might not be here to write this article and some of us might not be here to read it if it weren’t for Sir Frederick Banting and Charles Best and their group of scientists discovering the use of insulin as treatment for diabetes mellitus. They gave us the gift of life in 1922 in Toronto. The world has changed quite a bit since then and although their discovery still remains a lifeline, the diabetes population has exploded beyond belief. Most recent numbers published in the 4th edition IDF Atlas in Montreal reveal that 285 million people worldwide live with diabetes and indications show that the numbers of people in low and middle income countries are increasing rapidly and that far more people of working age are being diagnosed with Type 2 diabetes than had been estimated.</p>]]></description>

<h3>Climbing Stairs and Wild Salmon: A Common Bond?</h3>

<br />When you live on the 7th floor of a 12 story building, riding the elevator up is an automatic response.&nbsp; About a month ago we had a power outage that affected the building for 4 days.&nbsp; It&rsquo;s an older building and someone on the 12th floor decided to have <a href="http://www.energysavers.gov/your_home/space_heating_cooling/index.cfm/mytopic=12590" target="_blank">radiant flooring</a> installed.&nbsp; It knocked out the building&rsquo;s juice.&nbsp; So, instead of the usual thoughtless hopping on the elevator, the tenants of the house were struck with the fact that we actually had to rely on our bodies to carry us to our apartments.&nbsp; Being the type of building we are, most of us made the best of a potentially chaotic situation.&nbsp; We formed a group of floor captains (I was one) to see if any older or sickly people needed help or groceries.&nbsp; We lit battery operated candles in the stairwells, and on some floors people played music on portable radios, to make the hike more pleasant and give it some rhythm.&nbsp; At the end of the 4 days when things returned to normal and the elevators were up and running, many of us realized the benefits earned and learned from stair climbing.
